Make an application

How do I apply?

Take a Break is open to applications from 12 noon on Monday 10 May to 5.00pm on Monday 21 June 2021.

If you need some help applying for a Take a Break Scotland grant, please register for our short introductory course.

To apply online register your details and you will be provided with a link and pin to access the application form. This can take up to 30 minutes to come through, please remember to check your junk mail.

You can download an application form to print, complete and return to us.

First time you have applied?
If this is the first time you have applied to us you must send us supporting information with your application.
This can be any one of the following:

  • The most recent copy of your child’s Plan showing the level and type of support they are given at school
  • Co-ordinated Support Plan (CSP), Additional Support Plan (ASP) or Individual Education Plan (IEP)
  • Letter from school outlining your child’s current difficulties and level and type of support given
  • Recent diagnostic report or Educational Psychologist Assessment
  • Supporting letter from a third-party professional who knows your child in a professional capacity who can confirm your child’s current difficulties and the level and type of support they are given
  • A copy of your child’s current Disability Living Allowance (DLA) or Personal Independence Payment (PIP) award notice.

If you are a young person applying in your own right for the first time, please send us either:

  • Any of the above
  • A copy of your current Disability Living Allowance (DLA) or Personal Independence Payment (PIP) award notice if you are getting it.

If you have applied before, please send us:

  •  A copy of your child’s current Disability Living Allowance (DLA) or Personal Independence Payment (PIP)
  • Feedback from your last grant telling us how you used it and the difference it made to you/ your family if you have not already done so. You can give feedback here.

Remember: It is important to give as much detail as possible in your application form. Our decision will be based on the information you have given in the form alongside the supporting information you have sent us. Unfortunately, due to our limited resources we are not able to consider applications with incomplete or missing supporting information.
